Our Patient Intake Process

We’ve designed our intake process to be simple and supportive. New patients must complete a patient evaluation form covering their history, current concerns, medications, and other information. This helps us identify the right provider and ensure your insurance compatibility. Simply download the New Patient Evaluation form, complete it, and send it to us via email, fax, or mail.

Once scheduled, your first appointment will last at least an hour, during which we’ll dive into your history, discuss your goals, and develop a plan tailored to your needs. From there, we’ll collaborate with you on the best steps forward, whether that involves ongoing management, specialty treatments, or another care option.

Life circumstances sometimes require evaluations outside of traditional patient care. Our team provides professional, unbiased assessments for court-ordered psychiatric evaluations, disability determinations, weight-loss surgery clearances, and other major surgical evaluations. These independent evaluations are thorough and impartial, ensuring decisions are made with the patient’s and community’s best interests at heart.

Unlike traditional evaluations, independent assessments do not create a physician-patient relationship. Instead, they provide critical insights for legal, medical, or personal needs. Whether determining capacity for parenting in custody cases or assessing fitness for a procedure, we approach every case with compassion and diligence.
